The Step Up step Down- Intermediate Care Centre is a residential unit operated by Dublin Simon Community and Safetynet Primary Care with support from key collaborators including Housing First, HSE ACCESS Mental Health, Inclusion Health teams at St James’s Hospital and Mater Misericordiae University Hospital Dublin. The service is funded and supported by HSE Social Inclusion and the Department of Health.
The purpose of the new 12 bed unit at Dublin Simon Community`s Ushers Island Complex will be to provide short term semi acute healthcare (medical and nursing) interventions, treatment, investigations, observations for people without homes and who fit the below eligibility criteria.
Referrals can only be made by clinicians (homeless sector GPs or Nurses, GPs in addiction clinics or community GPs. The Multi Disciplinary Team meetings held at Merchant’s Quay Ireland weekly will also be utilised to identify suitable referrals. The referral form can be found here.

Male and female over the age of 18 years
Medical Conditions that are resolvable/stabilised within under 3 weeks. Applies to mental and physical health conditions
Pre inpatient work up / preparation
Observation / convalescence
Recuperation post hospital discharge

We ask homeless agencies to help ensure that clients referred to the service will have their bed kept for the short duration of their stay in the unit.
