Step Up Step Down Unit

The purpose of the Step Up Step Down Unit is to provide client-based medical care. Step Up Admissions prepare clients for a hospital stay or help prevent a hospital admission by addressing medical issues. Step Down Admissions help clients recuperate after a hospital stay and receive follow on care.

The Step Up Step Down Unit provides an integrated solution to the complex and growing needs of our clients and is located in our Health and Addiction Care Facility at Usher’s Island.

What We Do

Our unit provides short-term nursing and medical care for individuals without homes, offering treatment for 1 to 3 weeks based on each person's needs. We support clients who are active substance users while participating in our services, in addition, we also help and support those who want to reduce or discontinue use.

Personalised Care Plan

Each client benefits from a personalised care plan developed in collaboration with them. Our service includes voluntary private counselling and engaging group activities. Clients also receive assistance with applying for medical cards, securing accommodations, and accessing recovery services if desired.

The SUSD team includes a Nurse Manager, Nurses, a Supervisor, a Project Worker, and Support Workers, all trained to facilitate healing and engage clients in activities that promote well-being.
Our facility is staffed 24/7 with nurses and support workers. We also have a GP visit our facility Monday through Friday in the afternoon.
Each client benefits from a personalised care plan developed in collaboration with them.

We aim to address acute medical issues, empower clients to actively participate in their healthcare journey, and foster a healing environment.
Eligibility Criteria (Medical SUSD)

· Over the age of 18 years

· Registered as homeless

· Recent occurrence of acute illness

· The reason for admission and associated medical issues are expected to be resolved or stabilised within a three-week stay

· Post hospital discharge recuperation

· Stabilisation for clients with psychiatric / mental health issues

· Need for medication management/ establishment

· Referrals can only be made by clinicians (homeless sector GPs or Nurses, GPs in addiction clinics or community GPs
