Low Support Housing:
Hazelwood House provides long term accommodation to homeless single men and women who have low support needs. The project attempts to address the issues brought about by long term institutionalisation and addiction. Many of the residents work or participate in training projects during the daytime.
Referral Criteria
We accept referrals:
- from homeless services only
- for homeless single persons over 18 yrs

Services
The project operates a case management programme which involves carrying out an assessment of the needs of each individual in order to identify key areas where support is required. This is then used by the service user and keyworker to agree an action plan. Volunteers are also on hand to help with any needs that arise.
Staff currently provide a continuum of care and support in the following areas:
- Referrals to alcohol support workers
- Links with local health services (O/T; CPN; etc.)
- Referrals to resettlement
- Referrals to education programmes
Continuum of Care
Those in low support housing have often been referred to their current accommodation through the Dublin Simon Resettlement team or are in touch with them if they are interested in further move on options. Residents are also involved with the Learning and Development Team, taking classes and courses in areas that are of interest to them.
